WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISK
I know that running, walking and volunteering in an organized run/walk is a potentially hazardous activity and that I should not participate unless I am medically able and properly trained. I understand that I am solely responsible for my health and safety in connection with the event, and I acknowledge that I am physically capable of participating in and completing the event. I assume all risks associated with participating in this event including but not limited to, falls, contact with other participants, the effects of the weather, including high heat and/or humidity or other weather-related conditions, and the conditions of the race route, all such risks being known and appreciated by me. Having read this waiver and knowing these facts and in consideration of your accepting my registration entry, I, for myself and anyone entitled to act on my behalf, waive and release the organizers of the Hope Springs Eternal: WBCS Racing for Research at MCW, WBCS, the Medical College of Wisconsin, the Milwaukee County Parks, and all event sponsors, whether named or unnamed, together with their agents, representatives and successors, and any other organizations, entities, and individuals who are serving the Hope Springs Eternal: WBCS Racing for Research at MCW including all volunteers assisting in any capacity, from all claims or liabilities of any kind which I sustain and/or cause during my participation in this event or which is in any way related to or arises out of my participation in this event. Further, I grant permission of all the foregoing to use any photographs, motion pictures, recordings and any other record of this event which includes my picture, image, name or other information of any kind that may reveal my likeness or identity, for any legitimate purpose.
